Tommy deals with Sean losing his health coverage and the hospital shoving him into the mentally disturbed ward. Jimmy convinces Kevin to bail him out of prison, despite Tommy's accurate predictions of what Jimmy will do once he's out. Tommy deals with his guilt about taking the lives of Sal and Huey, even if it was to save his brother.

This episode was originally supposed to be the real third episode, however NBC decided to air "The World Will Break Your Heart" as the third and released this episode online on 2007-03-05. On NBC's website is was labeled as "too explosive for network TV" and was only available to US residents.
